Talented and Gifted Nomination Form 

South O'Brien Community Schools’ TAG program, for grades TK-12, identifies students in the following areas – specific academic strength or general intellectual ability. Nominees should show the ability or potential of going beyond the requirements of the regular classroom.

This nomination form is the initial component of the identification process. Should you wish to nominate a TK-12 South O’Brien student/resident, please complete the form and return it to the office or TAG coordinator.  Please remember that this individual needs to have demonstrated, or shows potential of demonstrating, ability higher than that of their peers. Upon nomination, the student will be screened for TAG qualification. (A teacher, parent, community member, peer, or the student may complete this form. Peer and self- nominations are accepted in grades 7-12 only.)  

Students currently in the TAG program do not need to be nominated. Completed nomination forms must be returned to the school office or Mr. White.
